If you have an appointment booked at Moorfields for an operation but are unable to attend, it is important that you let us know as soon as possible. This will enable us to allocate your appointment to someone else which makes a real difference to our resources.
If you ask for your operation to be rescheduled, we wil contact you in order to arrange a new date.
If you ask for your operation to be cancelled, you need do nothing else. We will contact your GP to let them know.

Hospital number: you will find your hospital number on your appointment letter. It is helpful if you can include this in all correspondence. Return to field (id:lbGS)
