If you are unable to attend your appointment, it is important that you contact the hospital as soon as possible. This will enable us to allocate the appointment to someone else which makes a real difference to our resources.

The more notice that you give us the better, but we ask for at least  three days notice to register and change your appointment.

Additional Help

Hospital number (if known): you will find your hospital number on your appointment letter. It is helpful if you can include this in all correspondence.Return to field (id:FOXx)

Clinic code (if known): You should be able to find the clinic code on your appointment card.Return to field (id:PJlc)

Consultant (if known): your consultant's name should appear on your appointment card and on any letters you have received from Moorfields.Return to field (id:xGIB)

Your date of birth: If you do not know your hospital number, please give us your date of birth, as this will enable us to find your patient records more easily.Return to field (id:2pmY)
